Qualifications

• Active Texas PTA license in good standing
• Strong clinical judgment and ability to work independently in the field under PT supervision
• Comfortable escalating condition changes and documenting rationale
• Strong written and verbal communication and documentation
• Reliable follow-through on treatment plans and goals
• Valid driver’s license and willingness to travel daily within assigned territory
• Ability to carry/transport basic therapy equipment
• Competency in core PTA scope: gait training, transfers, mobility, home safety, fall prevention

Responsibilities

• Deliver skilled PTA visits in the home under supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, and home safety.
• Set and track objective progress within PTA scope and report updates to PT.
• Coordinate with care team to manage changes in condition and orders.
• Coach patients and caregivers with practical home programs to promote adherence.
• Document promptly in Homecare Homebase (HCHB) with clarity and accuracy
