Job summary

Adaptive Home Health hires a Physical Therapist Assistant (PTA) to deliver skilled visits in patients' homes under a supervising PT. This full-time, field-based role offers a productivity-based compensation model and requires daily travel across the assigned Texas territory.

The PTA will focus on gait, balance, transfers, strengthening, home safety, and patient education, while documenting promptly in HCHB and coordinating with the care team for timely plan updates and optimal outcomes.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to supervising PT/RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on treatment plans and patient goals.
• Current driver’s license with daily travel across assigned territory.
• Ability to transport basic therapy equipment as needed.
• Competency in core PTA scope: gait training, transfers, balance, home safety assessment, caregiver coaching.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, and functional mobility.
• Set/track progress within PTA scope and communicate updates to the supervising PT.
• Drive multidisciplinary coordination and keep the care team aligned.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
