Job summary

Active In-Home Therapy seeks a dedicated Home Health Physical Therapist Assistant to support licensed therapists in delivering personalized rehabilitation at patients' homes. You will assist with assessments, implement treatment plans, and help patients regain strength, mobility, and independence.

The role is PRN, with a W2 employment status and flexible scheduling to cover preferred areas. This position complements a full-time workflow and emphasizes compassionate, patient-centered care.

Qualifications

• Current state licensure as a Physical Therapist Assistant.
• Home health experience preferred but not required.
• Strong clinical and observational skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Assist in conducting assessments of patients' physical conditions under the supervision of a licensed PTA.
• Implement individualized treatment plans developed by the PTA.
• Provide hands-on therapeutic interventions to improve strength, flexibility, balance, and coordination.
• Educate patients and families on therapeutic exercises, mobility aids, and strategies for independent living.
• Monitor and document patients' progress and report changes to the supervising PTA.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and up-to-date patient records.
• Communicate effectively with patients, families, and caregivers to ensure a supportive environment.
