Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) to deliver skilled visits in patients' homes across Texas under the supervision of a PT.

We offer per-visit compensation at $65 for regular PTA visits, with a focus on gait, transfers, strengthening, home safety, and fall prevention. Must hold an active Texas PTA license, possess a valid driver's license, and be comfortable daily travel within assigned territory.

Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment and independent field work within PTA scope.
• Ability to escalate changes to PT/RN/MD with clear rationale.
• Strong communication and documentation skills.
• Current driver’s license and ability to travel daily within territory.
• Carry/transport basic therapy equipment as needed.
• Competency in core home health PTA duties (mobility, safety, etc).

Responsibilities

• Deliver skilled PTA visits in the home under supervising PT’s plan of care.
• Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.
• Drive multidisciplinary coordination: elevate changes in condition, align on orders, and keep the care team moving.
• Coach patients and caregivers with practical home programs that actually get done.
• Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
