Job summary

Compassus is seeking a Home Health Physical Therapist Assistant to work one-on-one with patients in their homes, helping them regain strength and independence for daily activities.

You will provide PTA services, assess mobility, implement home exercise programs, and collaborate with a care team to ensure safe, high-quality care. A state license, CPR, and TB/Hepatitis screening are required, with at least one year in home health preferred.

Qualifications

• Bachelor/Associate Degree from an accredited PTA program.
• Currently licensed in the state of practice.
• Current CPR, TB screen and Hepatitis consent/declination.
• Minimum one year experience in a home health setting.

Responsibilities

• Provide PTA services one-on-one with home health patients.
• Assess mobility, develop and monitor home exercise programs.
• Maintain accurate documentation and ensure safety during visits.
