Job summary

Adaptive Home Health, LLC is seeking a Physical Therapist Assistant (PTA) for home health in Texas. You will deliver visits in patients' homes under PT supervision, focusing on gait, transfers, strengthening, and home safety to drive measurable outcomes.

Qualifications

• Active Texas PTA license in good standing.
• Ability to work independently in the field under PT supervision.
• Strong communication and documentation skills.
• Current driver’s license and willingness to travel across territory.
• Competence in core home health PTA scope: transfers, gait training, balance, home safety.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT's plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, home safety, and fall prevention.
• Set/track objective progress and communicate updates to the supervising PT.
• Drive multidisciplinary coordination with care team to adapt to condition changes and orders.
• Coach patients and caregivers with practical home programs that get done.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
