Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant to deliver skilled PTA visits in the home across Texas, under a supervising PT. The role emphasizes functional mobility, gait and balance training, transfers, home safety, and fall prevention to improve patient outcomes.

You will use Homecare Homebase (HCHB) for documentation, coordinate with the PT and care team, and coach patients and caregivers on practical home programs. Texas PTA license in good standing is required.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and independence in the field under PT supervision.
• Ability to escalate changes and document rationale clearly.
• Strong communication and documentation skills.
• Reliable follow-through on treatment plans and visits.
• Valid driver’s license and willingness to travel daily within assigned territory.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Implement treatment plans focusing on gait/balance, transfers, strengthening, and home safety.
• Set and track objective progress and communicate updates to the PT.
• Coordinate with the multidisciplinary care team to keep care on track.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
