Qualifikationen

• Active license to practice physical therapy in the state of assignment.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of related physical therapy experience in home health care or equivalent; mentoring/residency required if <1 year.
• Current CPR certification.

Aufgaben

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as needed, and perform discharge planning.
• Collaborate with patients, caregivers and interdisciplinary care team to support recovery and improve safety at home.
