Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist for visits in Annapolis, Maryland. This part-time or per diem role includes visiting adult and geriatric clients in Central and Southern Anne Arundel County, with a requirement to see patients at least two days per week between 8:30 am and 5 pm.

The PT will perform home health visits, develop individualized care plans, educate clients and families, and document care using EMR tools.

Qualifications

• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, balance, strength, and gait.
• Develop and implement individualized care plans with therapeutic exercises and gait training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
