Job summary

BAYADA Home Health Care, Inc. is seeking a full-time Home Health Physical Therapist in Crownsville, Maryland to deliver compassionate, skills-based rehabilitative care to clients in their homes within Anne Arundel County.

The role requires clinical excellence, independence, and strong teamwork with a multidisciplinary staff. The PT will conduct assessments, develop care plans, supervise activities, and document progress while ensuring safe, effective practices and adherence to BAYADA’s standards

Qualifications

• MD/US state PT license required (Maryland).
• Minimum 1 year of recent PT experience.
• Graduate from an APTA-approved PT program or AMA-approved equivalent.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility, gait, and balance assessments and wound care as needed.
• Develop and revise individualized care plans with therapy exercises.
• Educate clients and families on rehab activities to promote independence.
• Document care observations and interventions clearly in EMR.
• Collaborate with nursing for multidisciplinary care planning.
