Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for a part-time or per diem role to perform home health visits across Carroll County, Maryland.

The PT will work in adult and geriatric populations, conducting assessments and implementing individualized plans of care in patients’ homes. Responsibilities include performing diagnostic tests, wound care, gait and balance training, and education for patients and families.

Qualifications

• Maryland PT license required.
• Minimum of 1 year PT experience.
• Graduation from an APTA-approved program or equivalent.

Responsibilities

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance).
• Implement wound care and protective interventions.
• Develop and revise individualized care plans with PT interventions.
• Educate clients and families on rehabilitative care and safety at home.
• Accurately document observations and interventions in EMR.
