Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for a part-time or per diem position serving seniors in Northern Baltimore County, Maryland. The role involves home visits, conducting assessments, and implementing rehabilitation plans to help clients stay at home.

The PT will work with a multidisciplinary team, providing care during mornings/evenings or weekends. A Maryland PT license and one year of PT experience are required, with emphasis on independent work and

Qualifications

Maryland PT license required.
Minimum of one year PT experience.
Graduation from an APTA-approved program.
Ability to work independently and manage time.
Strong interpersonal and organizational skills.
Experience with EMR systems preferred.

Responsibilities

Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements related to mobility, gait, strength, balance and safety.
Implement wound care interventions using evidence-based dressings and topical agents.
Develop and implement individualized care plans including manual therapies and gait training.
Continually assess and revise the PT care plan in collaboration with nursing and other care team members.
Educate clients and family members on rehabilitative care and activities for safety and independent living.
