Job summary

BAYADA Home Health Care is hiring a Home Health Physical Therapist in Harmans, MD for part-time or per diem home visits across Central and Southern Anne Arundel County. You will perform PT assessments, develop care plans, and collaborate with a multidisciplinary team to support clients to remain at home.

The role requires MD PT licensure, strong time management, and comfort with EMR documentation. Flexible scheduling and competitive compensation per visit are offered.

Qualifications

• Current Maryland Physical Therapist license.
• Graduation from APTA-approved program or equivalent per certification.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility, strength, balance, and safety assessments.
• Develop and implement individualized care plans with PT interventions.
• Assess and revise care plan in multidisciplinary team.
• Educate clients and family members on rehabilitative care.
• Document observations and interventions using electronic records.
