BAYADA Home Health Care in Maryland is seeking an experienced Physical Therapist (PT) for a full-time role performing home health visits across Central Anne Arundel County. You will be an integral member of a multidisciplinary team delivering rehabilitative care and skilled nursing to help clients remain at home.

Qualifications

• Licensed Physical Therapist in Maryland.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.
• EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territory.
• Perform mobility, strength, balance, and safety assessments.
• Implement wound care and protective interventions.
• Develop and adjust individualized care plans with therapy interventions.
• Educate clients and caregivers to promote independence.
• Document observations and interventions in an EMR touchpad tablet.
