Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist for Severn, Maryland. The role is part-time or per diem, performing home health visits for adult and geriatric clients in Central and Southern Anne Arundel County.

Please note the candidate must be available to see patients a minimum of two days per week, Monday through Friday, between 8:30am–5pm. BAYADA offers a supportive team environment with PPE provisions and ongoing training.

Qualifications

• Maryland Physical Therapist license required.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
• Develop and implement individualized care plans including manual therapy, gait training, and balance exercises.
• continually assess and revise the PT care plan in collaboration with nursing.
• Educate clients and families on rehabilitative care to promote health and independence.
• Accurately document observations and evaluations in the EMR.
