Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist for the Glen Burnie, Maryland area.

This part-time or per diem opportunity involves conducting home health visits for adult and geriatric clients in Central and Southern Anne Arundel County, with a minimum of two days per week. The PT will join a multi-disciplinary team to provide rehabilitative care and skilled nursing, maintaining patients at home and delivering individualized care plans.

Qualifications

• Maryland Physical Therapist license required.
• Experience with EMR systems preferred.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility, joint range of motion, transfer status, balance, and strength.
• Develop and implement individualized care plans including exercises and gait training.
• Continually assess and revise the care plan with nursing in the multidisciplinary team.
• Educate clients and family members on rehabilitative care and activities.
• Document observations and interventions using an electronic touch pad tablet.
