Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist to provide patient-centered care in Catonsville, Maryland. The role offers full‑time, part‑time, or per diem opportunities across Howard County and surrounding areas, with visits scheduled 8:30a–5p, Monday through Friday.

You’ll conduct assessments, develop individualized plans, and collaborate with a multidisciplinary team to support adult and geriatric patients in their homes.

Qualifications

• A current Maryland Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduation from a program approved by the APTA or AMA committee.
• Ability to work independently and manage time effectively.
• Strong interpersonal and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements as described.
• Develop and implement individualized care plans and therapeutic exercises.
• Continuously assess and revise care plans with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and safe living.
• Document observations and interventions using a state-of-the-art touch pad tablet.
