Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to assess patient needs, develop and implement a plan of care, and monitor progress in patients' homes. This role emphasizes mobility, endurance, pain management, and improving overall well-being.

Responsibilities include evaluating functional status, coordinating with care teams, educating patients and families, and documenting visits. A current PT license and reliable transportation are required; CPR is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire (preferred).

Responsibilities

• Evaluate patient's functional status and PT needs; assist in plan of care.
• Provide PT services within licensed scope and plan of care.
• Observe patient response and report changes to supervisor/physician.
• Treat to relieve pain and restore function; maintain maximum functioning.
• Educate patient and family about plan of care and progress.
• Visit patients according to Plan of Treatment and document progress.
• Complete PT evaluations and develop plan of care per state guidelines.
• Report outcomes and projected frequency of care.
• Provide care in diverse home settings with nondiscrimination.
