Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient's functional status and physical therapy needs; assists physician in developing therapy plan of care.
• Provides PT services within the scope of practice as defined by state laws and plan of care, in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
• Visits patients per Plan of Treatment; completes progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
