Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health, performing assessments, developing care plans, and delivering interventions to improve mobility and function in patients in home settings.

Responsibilities include evaluating functional status, collaborating with physicians and nurses, documenting progress, and ensuring care aligns with state guidelines. A valid PT license and reliable transportation are required; CPR is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Valid driver's license and access to a dependable vehicle or public transportation.
• CPR certification preferred.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the health care team.
• Observes and reports the patient's response to treatment and changes in condition to the supervising staff.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs patient, family, and care team regarding plan of care and progress toward goals.
• Visits patients according to Plan of Treatment, completes progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
