Job summary

UnitedHealth Group is hiring a Physical Therapist for Home Health to assess and treat patients, develop a plan of care, and coordinate with physicians and nurses. You will evaluate functional status, pain, and mobility, then implement interventions to improve well-being and progress toward goals.

The role requires current PT licensure, reliable transportation, and CPR certification or the ability to obtain it within 90 days.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs and assists in developing the therapy plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient response to treatment and changes to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Informs patients, families, and care team about plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Adaptability to diverse patient situations.
