Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health. You will assess patient mobility, endurance, pain, and functional needs, then create and implement a plan of care to improve well‑being.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
• Preferred CPR certification or ability to complete within 90 days of hire

Responsibilities

• Evaluates the patient's functional status and PT needs and assists in care planning.
• Provides PT services within the plan of care and state practice laws.
• Observes and reports patient responses to treatment to the nurse/physician.
• Treats patients to relieve pain, restore function, and maximize functioning per standards.
• Instructs patient and family about plan of care, limitations, and progress.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops PT plan of care following state guidelines.
• Reports outcomes, goals, and projected frequency of care; works in diverse settings.
