Job summary

UnitedHealth Group is seeking a Physical Therapist to perform home health assessments and create treatment plans to improve patient well-being. You will deliver interventions, monitor progress, and coordinate with physicians and care teams to achieve goals.

The role requires current PT licensure in the practicing state and a valid driver's license with access to reliable transportation. A CPR certification is preferred or can be obtained within 90 days of hire.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports patient responses to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient, family, and care team about the plan of care, limitations, and progress.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes PT evaluations and develops the plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
• Works in diverse home settings regardless of age, race, creed, or disability
