Job summary

UnitedHealth Group is seeking a Physical Therapist in Home Health to assess and evaluate patient care needs related to mobility, endurance, pain, and other conditions, and to help develop and implement a treatment plan.

You will perform interventions to improve well-being, monitor progress, and communicate with patients, families, and care teams. The pay includes hourly and per-visit components, with comprehensive benefits and stock/401k options.

Qualifications

• Current Physical Therapy licensure in state of practice
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
• Current CPR certification or ability to complete within 90 days of hire

Responsibilities

• Evaluates patient functional status and therapy needs
• Assists physician in development of therapy plan of care
• Provides PT services within scope of practice
• Observes and reports patient response to treatment
• Instructs patient and family/caregivers on plan of care and progress
• Visits patients per Plan of Treatment and submits progress notes
• Completes evaluations and develops PT plan of care per state guidelines
• Reports outcomes and planned frequency of care
