Job summary

Powerback Rehabilitation is seeking a Physical Therapist to provide in-home outpatient rehabilitation for older adults, evaluating needs and delivering personalized PT programs in the patient’s home or living facilities. You will educate patients and caregivers to promote independence and coordinate care with the rehabilitation team.

Eligible candidates hold a PT license, have a Master’s (or PT-related Master’s with a PT Bachelor’s), and possess transportation to patient residences.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice
• Master’s degree in Physical Therapy (or Master’s in PT-related field + Bachelor’s in PT)
• Valid driver's license or reliable transportation to patient residences

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults
• Assess needs, develop and deliver PT programs to facilitate rehabilitation
• Educate patients and caregivers to promote independence and functional improvements
• Supervise PT assistants and students as allowed by state licensure
• Manage documentation and scheduling to ensure timely care and compliance
