Job summary

Powerback Rehabilitation to You seeks a licensed Physical Therapist to provide in-home outpatient rehabilitation services to older adults. You will evaluate, diagnose, and implement individualized PT programs in the patient’s home or in living facilities to maximize independence.

You will work with a multidisciplinary team, educate patients and caregivers, and ensure timely documentation and adherence to care plans. The role offers a flexible schedule and opportunity to serve across communities.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or PT-related Master’s with a Bachelor's in PT.
• Driver's license or reliable transportation to patients' residences.
• The qualifying PT degree earned outside of the U.S. must be evaluated for U.S. equivalence.

Responsibilities

• Provide in-home outpatient physical therapy evaluations and treatment.
• Develop and deliver individualized therapy plans to restore function.
• Coordinate with the rehabilitation team to refer to other disciplines as needed.
• Educate patients and caregivers to promote independence.
• Document care and schedule adherence and participate in team meetings.
