Job summary

VitalCaring Group seeks a Home Health Physical Therapist to deliver patient-centered care in patients’ homes across Florida. You will evaluate mobility, develop care plans with the care team, and provide hands-on therapy to improve independence and function.

Role emphasizes autonomy, collaboration with nurses and physicians, and timely EMR documentation. Requires DPT or Master’s, active PT license, and 1–2 years home health experience plus CPR certification.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
