Job summary

VitalCaring is hiring a Home Health Physical Therapist to deliver patient-centered care in patients' homes. You will evaluate mobility, develop individualized plans, and provide hands-on therapy while educating patients and caregivers.

The role emphasizes autonomy, collaboration with an interdisciplinary team, and timely EMR documentation. Requirements include DPT or MS in PT, current state license, and 1–2 years of home health experience.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver patient-centered home health PT care.
• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor progress and adjust treatment plans; support discharge planning.
• Supervise PTAs as allowed by state guidelines.
• Document care and progress accurately within the EMR.
• Collaborate with nurses, therapists, and physicians for coordinated care.
