Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings across Florida. You will evaluate, plan, and provide hands-on therapy to help patients regain mobility and independence.

You’ll lead treatment plans in collaboration with physicians, educate patients and families, and ensure safe transitions of care while documenting progress in the EMR.

Qualifications

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.

• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately and timely within the EMR.
