Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to help patients recover mobility and regain independence in their own homes. You will work autonomously within a compassionate care team, delivering personalized, evidence-based therapy and education to patients and families.

Ideal candidates have 1–2 years of home health experience, hold a current PT license in Florida, and bring strong assessment, communication, and clinical skills.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Document patient progress and care in the EMR and participate in team meetings.
