Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to provide patient-centered care in patients' homes across Florida. You will evaluate mobility, develop individualized plans with physicians, and deliver hands-on therapy to improve function and independence.

Autonomy and relationship-building are central as you supervise PTAs and coordinate with nurses, therapists, and physicians to ensure seamless care. Home visits require a valid PT license, CPR, and reliable transportation.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1-2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner closely with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
