Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver patient-centered care in patients’ homes. You will assess mobility, develop individualized plans, and provide hands-on therapy to restore independence and function.

The role emphasizes autonomy, collaboration with a care team, and ongoing patient and family education. A valid PT license and CPR certification are required, with 1–2 years of home health experience preferred. We value compassionate, outcomes-driven clinicians.

Qualifications

• Graduate of accredited PT program (DPT or Master’s) and licensed in state.
• Minimum 1–2 years of home health clinical experience.
• Strong assessment, critical thinking, and patient education skills.
• Current CPR certification and valid driver’s license.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehab potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.
