Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings, focusing on safe mobility, independence, and functional outcomes. You will work autonomously within a collaborative care team, guiding patients and families to sustain improvements in daily living at home.

Requirements include a DPT or Master’s in Physical Therapy, active PT license, and 1–2 years of home health experience.

Qualifications

• Current PT license in good standing in the states of practice.
• Minimum 1–2 years of clinical experience, home health preferred.
• Excellent communication and patient education abilities.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress timely within the EMR; participate in team meetings.
