Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings across Florida. You will assess mobility, develop individualized plans in collaboration with physicians, and provide hands-on therapy to improve functional outcomes.

This role emphasizes autonomy, trust, and direct impact on patients’ independence. Ideal candidates have 1–2 years in home health, hold an active PT license, and bring strong communication and teaching skills.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Coordinate transitions of care and contribute to discharge planning.
• Document patient care and progress accurately in the EMR and participate in team meetings.
