Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care at patients’ homes in Florida. You will assess functional status, develop and implement individualized plans with physicians, and provide hands-on therapy to improve mobility, balance, and independence.

The role emphasizes autonomy, ongoing patient education, and collaboration with nurses, therapists, and physicians to ensure coordinated care. A valid PT license, CPR, and a driver’s license are required.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status, mobility, strength, and rehab potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor progress and adjust treatment plans as needed.
• Support safe transitions of care and discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner with nurses, therapists, and physicians for coordinated care.
• Document patient care and progress in the EMR.
• Participate in case conferences and team meetings.
