Job summary

Adara Home Health is seeking a Physical Therapist Case Manager to oversee home health services in the Rochester, MN area, including Cannon Falls, Red Wing, and Lake City. You will collaborate with families and healthcare teams to develop goal-centered plans and provide skilled care with clinical oversight.

The role requires a Minnesota PT license, 2+ years of healthcare experience, and the ability to manage a flexible schedule.

Qualifications

• Must be current Minnesota-licensed PT without restrictions.
• Must pass TB testing (Mantoux, TB blood test or chest x-ray).
• Must have access to reliable automobile for travel to client homes.
• Must hold valid driver’s license and auto insurance when using personal vehicle.
• U.S. Citizenship or valid Alien Work Permit required for employment.
• Disclose criminal history related to health services; convictions may disqualify.
• Complete background checks with MDH and OIG as applicable.

Responsibilities

• Assess clients to determine care needs and implement plans.
• Collaborate with physicians and home health team to update care plans every 60 days.
• Advocate for clients, assess home safety, and implement abuse prevention plans.
• Ensure timely and compliant documentation per state and payer requirements.
• Educate clients, families, and caregivers; provide guidance to staff.
• Provide direct care within Minnesota PT Practice Act guidelines.
• Coordinate with family, caregivers, and community resources.
• Provide clinical oversight to licensed and unlicensed staff.
• Manage financial aspects of home health services.
• Maintain flexible scheduling and adapt to changes.
