Job summary

Adara Home Health is seeking a PT Case Manager to deliver and oversee home health services for clients in the Hutchinson, MN area, traveling to homes for 1:1 care. You will collaborate with families and the care team to implement goal-centered plans with attention to quality and financial considerations.

The role requires MN-licensed PT, ability to manage a flexible schedule, and clinical leadership for licensed and unlicensed staff.

Qualifications

• Must be licensed as a Physical Therapist in MN with active status.
• Must pass background checks including MDH and OIG lists.
• Access to reliable car for travel to client homes.
• Valid driver’s license and car insurance when using personal vehicle.

Responsibilities

• Assess and plan client care and implement goale-centered plans.
• Provide 1:1 client care in homes; ensure safety and quality care.
• Coordinate with physicians and home health team; update plans every 60 days.
• Provide education to client, family, and caregivers; document accurately.
