Job summary

Adara Home Health, Inc. is seeking a Physical Therapist Case Manager to deliver and oversee home health services in the Hutchinson area, with travel to Waconia and Chaska. You will collaborate with families and the care team to implement goal-centered plans and provide skilled clinical oversight.

The role requires a MN PT license, clinical leadership, and strong communication. Base salary ranges with productivity bonuses and comprehensive benefits are offered.

Qualifications

• Licensed as a Physical Therapist in Minnesota.
• Proven ability to assess and plan care with accuracy.
• Background checks and health screenings as required.

Responsibilities

• Performs initial and periodic assessment of clients to determine care needs and evaluates effectiveness of care.
• Collaborates with physicians and home health team to develop/update the plan of care at least every 60 days.
• Advocates for client safety by assessing home environment and creating abuse prevention plans.
• Maintains documentation standards and ensures compliance with state and payer requirements.
• Educates clients, families, caregivers, and licensed/unlicensed staff as needed.
• Delivers direct care per Minnesota Physical Therapy Practice Act and coordinates with care team.
• Oversees and delegates to licensed and unlicensed personnel.
• Ensures appropriate financial management of home health services.
• Manages schedule and adapts to changes.
