Adara Home Health, Inc. is seeking a Physical Therapist Case Manager in Minnesota to oversee home health services and develop goal-centered care plans with clients and families.

You will collaborate with the home health team to deliver skilled care and provide clinical oversight to staff, while managing a 1:1 client care schedule in clients' homes. The role requires a Minnesota PT license, ability to travel, and a commitment to safety, documentation, and reimbursement considerations.

Qualifikationen

• Licensed PT in Minnesota (no license limitations).
• Background checks including MN Dept of Health and OIG lists will be completed.
• Access to dependable automobile for travel to client homes.
• Valid driver’s license and auto insurance for company travel.

Aufgaben

• Assess client care needs and develop care plans.
• Update home health plan of care at least every 60 days.
• Collaborate with physicians and home health team for care delivery.
• Provide education to clients, families, and caregivers.
• Ensure safety and documentation compliance per state regulations.
• Coordinate care with families, caregivers, and community resources.
• Provide clinical oversight to licensed and unlicensed staff.
