Job summary

Adara Home Health, Inc. is seeking a Physical Therapist Case Manager to provide and oversee home health services in Mora, MN and surrounding areas. The role requires collaboration with the client, family, and care team to implement goal-centered plans and ensure quality and financial considerations are met.

The position is full-time, Monday–Friday, with travel to client homes for 1:1 care. Competitive base salary ($80,000–$115,000) plus productivity bonuses and comprehensive benefits are offered.

Qualifications

• Licensed PT in Minnesota with no license restrictions.
• TB testing (Mantoux or equivalent) required.
• Reliable automobile and valid driver’s license with insurance.
• U.S. Citizenship or valid Alien Work Permit.
• No convictions related to health services; background checks required.
• CPR certification is highly recommended.

Responsibilities

• Assess clients to determine care needs and develop care plans.
• Collaborate with physicians and the home health team to update plans.
• Provide clinical oversight to licensed/unlicensed staff.
• Ensure timely and compliant documentation for state and payer requirements.
• Educate clients, families, and caregivers about care plans.
• Coordinate client care with family and community resources.
• Manage scheduling and provide 1:1 client visits in homes.
