Job summary

Adara Home Health seeks a dedicated Occupational Therapist Case Manager to deliver skilled home-based therapy throughout Rochester, MN. This full-time role requires assessing client needs, planning treatments, and coordinating with physicians.

You will travel to client homes, perform 1:1 care, and manage a flexible schedule within office hours. Qualified candidates are licensed OT professionals in Minnesota with 2+ years in health care; home care experience preferred.

Qualifications

• Licensed as an Occupational Therapist in Minnesota
• Graduate of AOTA approved school and currently certified by AOTA
• At least 2 years experience in health care; home care preferred
• Strong interpersonal communication and teaching skills
• Ability to work independently and organize work effectively
• Access to dependable vehicle or reliable transportation for travel to multiple client homes

Responsibilities

• Perform initial and ongoing assessments to determine care needs and plan interventions
• Provide OT treatments and evaluations under physician guidance
• Translate exercises into daily living activities and provide equipment guidance
• Maintain records and progress notes; report client responses to physician and Area Clinical Manager
• Instruct and supervise COTAs, HHAs, LPNs and RNs in therapy programs
• Assist families with information and support to motivate client progress
• Maintain confidentiality and ensure safety of clients and staff
• Pursue community resource development to meet client needs
