Job summary

Adara Home Health, Inc. seeks a qualified Physical Therapist Case Manager to deliver and supervise home health services in Minnesota. You will collaborate with physicians and the care team to create goals-based plans, provide direct client care, and ensure high-quality outcomes.

Travel to client homes is required, 1:1 care provided, with a schedule Monday–Friday. Competitive base salary plus productivity bonuses and comprehensive benefits. Reporting to Clinical Supervisor.

Qualifications

• Licensed Physical Therapist with Minnesota license in good standing.
• TB testing requirement and background checks as per policy.
• Access to dependable automobile and valid driver's license for travel.
• Proof of citizenship or work authorization in the U.S.

Responsibilities

• Provide and oversee home health services for clients in the community.
• Develop and implement goal-centered care plans with the health care team.
• Provide clinical oversight to licensed and unlicensed personnel.
• Ensure safety, documentation accuracy and regulatory compliance.
• Coordinate client care with families, caregivers, and resources.
• Manage scheduling and adapt to changes in workload.
