Job summary

Adara Home Health, Inc. is seeking a Physical Therapist Case Manager to provide and oversee home health services for clients in the Rochester, MN area. You will collaborate with the health care team, develop goal-centered care plans, and ensure care adheres to quality and financial considerations.

The role requires Minnesota PT licensure, two years in health care, and the ability to drive to client homes. Base salary ranges from $80,000 to $115,000 per year plus productivity and benefits.

Qualifications

• Two years of experience in a health care facility or Home Health Care.
• Licensed Physical Therapist in Minnesota with no limitations.
• Valid driver’s license and access to a dependable automobile.

Responsibilities

• Provide and oversee home health services per client plan.
• Collaborate with physicians and home health team to update plan of care every 60 days.
• Assess home environment for safety and implement abuse prevention plans.
• Ensure documentation meets state and payer requirements.
• Educate clients, families, and caregivers on care and safety.
• Supervise licensed and unlicensed personnel and delegate tasks.
• Coordinate care with family and community resources.
