Job summary

Adara Home Health in Hutchinson, MN is seeking an experienced RN Case Manager to deliver direct nursing care and oversee home health services for clients in the community. You will collaborate with clients, families, and the health care team to develop goal-centered care plans, ensuring quality and financial considerations are integrated.

This role reports to the Clinical Supervisor. The position is full-time with office hours, travel to client homes, and an on-call rotation.

Qualifications

• Must be currently licensed as an RN in Minnesota with no restrictions.
• Provide negative TB test results (TB testing).
• Must have access to a dependable automobile for client visits.
• Valid driver’s license and proof of auto insurance for business use.
• U.S. Citizenship or valid Alien Work Permit.
• Disclose any criminal history related to health services.
• Pass background screenings including MDH and OIG lists.

Responsibilities

• Perform initial and periodic client assessments to determine care needs and interventions.
• Develop and update home health plans of care with the care team, at least every 60 days.
• Provide clinical oversight and leadership to licensed and unlicensed staff.
• Ensure safety and reporting of abuse, neglect, or financial exploitation.
• Meet documentation standards and payer/billing requirements.
• Educate clients, families, caregivers, and staff on care plans and processes.
