Job summary

Adara Home Health is seeking a dedicated RN Case Manager to deliver direct home health nursing care and to provide clinical oversight for the care team. You will collaborate with clients, families, and the broader care team to implement goal-focused plans that balance quality care with financial considerations.

The role requires travel to client homes across the Buffalo, MN area, weekly office coordination, and participation in on-call rotations.

Qualifications

• RN license in Minnesota (active, no revocations)
• TB test or chest X-ray proof of TB screening
• Access to reliable vehicle for multiple client visits
• Valid driver’s license and car insurance when using personal vehicle
• US citizenship or eligible Alien Work Permit; background checks completed

Responsibilities

• Assess and plan client care with 60-day updates or as changes occur
• Provide skilled nursing care and clinical oversight
• Coordinate care with family, physicians, and home health team
• Ensure safety, documentation, and compliance with payer/billing requirements
• Educate clients, families, and caregivers on care plans and treatments
