Job summary

21st Century Home Health Services is seeking a Full Time Physical Therapist Case Manager to provide in-home evaluations and therapeutic care in Santa Cruz County. You will work with clinicians to develop and implement care plans aimed at restoring function and maximizing independence in patients’ home environments.

Responsibilities include evaluating patients, creating treatment plans, coordinating with physicians, and supervising PTAs as per regulations. A CA PT license and BLS/CPR are required.

Qualifications

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience in home health care preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.

Responsibilities

• Evaluate and address the needs of individuals in their homes to support safe living.
• Restore function and prevent disability through prescribed therapy.
• Evaluate and treat patients using medically prescribed PT programs; supervise PTAs as needed.
• Coordinate with physicians to ensure high-quality care and progress reporting.
