21st Century Home Health Services (21HHS) is seeking a Home Health Physical Therapist Case Manager to evaluate patients in their homes, develop and supervise treatment plans, and coordinate with physicians to ensure high-quality care. The role emphasizes autonomy, in-home visits, and interdisciplinary collaboration within a leading Bay Area home health organization.

Ideal candidates hold a CA PT license, BLS/CPR, and a driver's license, with experience in home health and a patient-centered

Qualifications

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Knowledge of development and culture as it relates to health care.

Responsibilities

• Perform an evaluation in the home and develop a care plan with PTAs to follow.
• Help patients regain mobility and independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients’ physicians to ensure a high level of care.
• Evaluate and report patients’ progress to MD.
