Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes across Florida. You will assess functional status, develop and execute individualized treatment plans, and promote independence and mobility.

Enjoy autonomy in a relationship-driven role, collaborating with nurses and physicians, documenting progress in the EMR, and guiding caregivers. A valid PT license in Florida and CPR certification are required, with 1–2 years of home health preferred.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice (Florida).
• Minimum 1–2 years of home health experience preferred.

Responsibilities

• Evaluate patients' functional status, mobility, strength and rehab potential.
• Develop and implement individualized care plans with physicians and the team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety.
• Monitor progress, adjust plans, and document in EMR.
• Coordinate transitions of care and contribute to discharge planning.
• Collaborate with nurses, therapists, and physicians for coordinated care.
• Supervise PTAs as per state guidelines and mentor staff.
