• DPT or Master’s in Physical Therapy (PT)
• Active Florida PT license
• Home health experience preferred
• CPR certification on file

Responsibilities

• Evaluate functional status, mobility, strength, and rehab potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans and safety strategies.
