Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes throughout Sebring, FL and nearby areas. You will evaluate function, design and implement individualized treatment plans, and educate patients and families to promote independence and safety.

We offer autonomy, collaboration with nurses and physicians, and ongoing clinical growth opportunities. The role requires a DPT or Master’s in PT, FL license, 1–2 years home health experience

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice (Florida).
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately and timely within the EMR.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
